Keto, Carnivore, and the War on Carbs: What the Research Actually Says

Somewhere between the Joe Rogan episodes and the Instagram transformation photos, carbohydrates became the villain of modern nutrition.

Not saturated fat. Not ultra-processed food. Not the 500 calorie surplus most people are eating without realizing it.

Carbs.

Specifically, the narrative goes like this: carbohydrates spike insulin, insulin stores fat, therefore carbohydrates make you fat, therefore eliminating carbohydrates is the key to losing fat and living forever. This idea has been packaged into keto, carnivore, low-carb, paleo, and approximately forty other named diets, each with its own book deal, supplement line, and legion of devoted followers who will argue with you about it at dinner parties.

So let's look at what the research actually says. Not the anecdotes. Not the before and after photos. The peer-reviewed, controlled, replicated research.

Why Keto and Low-Carb Diets Actually Work

Here's something the anti-carb crowd gets right: keto and low-carb diets do produce fat loss. For a lot of people they produce significant fat loss, at least initially. This is real and documented.

Here's what they get wrong: the reason why.

The insulin-fat storage theory — that carbohydrates uniquely drive fat gain through insulin response — has been tested directly in controlled metabolic ward studies where researchers control every calorie consumed and burned by participants. The results are consistent: when calories and protein are matched, low-carb diets do not produce meaningfully more fat loss than higher carb diets. The macronutrient composition, holding calories equal, does not determine the rate of fat loss.

What actually happens when people go keto or low-carb and lose weight is this: they eat less. Not because carbs are gone and insulin is suppressed. Because removing an entire macronutrient category from your diet is a highly effective way of accidentally reducing your total calorie intake. Bread, pasta, rice, potatoes, fruit — these foods make up a significant portion of most people's daily calories. Cut them out and your overall intake drops, often substantially, without you ever counting a calorie.

Fat loss follows. Not because of carb restriction specifically. Because of the calorie deficit that carb restriction happened to create.

This distinction matters enormously because it determines what you actually need to do — and more importantly, what you don't.

What the Long-Term Research Shows

Short-term, low-carb diets often outperform higher carb diets for weight loss in the first three to six months. This is well established and not particularly controversial. The initial advantage comes from a few sources: the calorie reduction mentioned above, a significant drop in water weight as glycogen stores deplete (glycogen holds water, and depleting it produces rapid scale movement that looks like fat loss), and for some people a genuine reduction in appetite from higher fat and protein intake.

The picture changes at twelve months and beyond.

Multiple large meta-analyses — studies that pool data across dozens of individual trials — show that by the one year mark, the difference in fat loss between low-carb and higher carb diets essentially disappears when total calorie intake is accounted for. A 2020 review published in the British Medical Journal analyzing 121 randomized controlled trials found that while low-carb diets showed slightly greater weight loss at six months, the difference was not clinically meaningful and largely vanished at twelve months.

The most honest summary of the long-term evidence: any diet that produces a sustained calorie deficit produces fat loss. The macronutrient composition is largely irrelevant to the outcome. What determines long-term success is adherence — whether you can actually sustain the approach for months and years without feeling miserable.

For some people, low-carb eating is genuinely sustainable and enjoyable. For others it's a form of slow torture that ends in a three-day pasta binge. Both responses are valid. Neither makes the person right or wrong about the underlying science.

The Performance Cost Nobody Talks About

Here's where the carb elimination argument falls apart most completely, and it's the part that gets the least attention in low-carb communities.

Carbohydrates are the primary fuel source for high-intensity exercise. Not fat. Not protein. Carbohydrates, stored as glycogen in your muscles and liver, are what your body preferentially burns when you're lifting weights, sprinting, or doing anything that requires significant power output.

This is not a philosophical position. It is basic exercise physiology.

When glycogen is depleted — which is exactly what happens on a ketogenic or very low-carb diet — your ability to perform high-intensity work is compromised. You can still exercise. You can still lift weights. But the research consistently shows reductions in power output, strength, and training volume in athletes and recreational exercisers who adopt very low-carb protocols, particularly in the initial weeks and months of adaptation.

For someone whose primary goal is fat loss and who is doing light to moderate exercise, this tradeoff might be acceptable. For someone trying to build muscle, get stronger, or perform at a meaningful level in any sport or activity that requires power, chronically depleting glycogen is working directly against that goal.

Muscle is built in the gym through progressive overload — lifting progressively heavier loads over time. That progressive overload requires the ability to train hard. Training hard requires fuel. Carbohydrates are that fuel.

This is why the carnivore influencer who looks impressive in photos is almost certainly not building that physique on a zero-carb diet — or if they are, they are a genetic outlier performing significantly below their potential. Elite strength and physique athletes, without exception, eat carbohydrates. This is not a coincidence.

So What Should You Actually Eat

The honest answer is: whatever sustainable eating pattern puts you in an appropriate calorie deficit for fat loss, or at maintenance or slight surplus for muscle building, while hitting your protein target.

If that happens to be low-carb because you genuinely prefer it, great. If it includes bread and rice and potatoes because those foods make your life more enjoyable and sustainable, also great. The research does not support the idea that one approach is categorically superior to the other for body composition when calories and protein are matched.

What the research does support, consistently and across decades of study:

Adequate protein — 0.7 to 1 gram per pound of bodyweight — is the single most important dietary variable for body composition, regardless of what you do with carbs and fat.

Total calorie intake determines whether you lose fat, gain weight, or stay the same. No dietary framework overrides this. Not keto. Not carnivore. Not intermittent fasting. Energy balance is the governing mechanism.

Sustainability determines long-term outcomes. The best diet is the one you can adhere to for months and years without making yourself miserable. A perfect low-carb plan abandoned at week eight produces worse results than a moderate mixed diet followed for a year.

The Real Reason the War on Carbs Won't End

If the evidence for carb elimination is this mixed, why does the narrative persist so powerfully?

Because it is extraordinarily profitable.

Keto and carnivore have generated entire industries — specialized food products, supplements, cookbooks, coaching programs, meal delivery services, testing kits. These industries have a financial interest in the narrative continuing. Every person who believes carbs are the enemy is a potential customer for a low-carb alternative product.

The people loudest about carb elimination online are almost universally selling something attached to it. This doesn't automatically make them wrong. But it does mean you should evaluate their claims with the same skepticism you'd apply to a pharmaceutical company funding its own drug trial.

The evidence doesn't support a war on carbs. It supports a clear-eyed understanding of calorie balance, adequate protein, and a sustainable eating pattern that you can actually maintain.

Rice is not the problem. Bread is not the problem. The problem has always been, and continues to be, a sustained calorie surplus — regardless of where those calories come from.

The Bottom Line

Keto works. Carnivore works. Low-carb works. They all work for the same reason: they tend to reduce total calorie intake. When they stop working, it's almost always because calorie intake crept back up, adherence broke down, or the initial water weight loss was mistaken for sustainable fat loss.

You do not need to eliminate carbohydrates to lose fat. You do not need to eliminate carbohydrates to build muscle — in fact, doing so likely makes building muscle harder. You need a calorie deficit, adequate protein, and a way of eating you can sustain without feeling like you're being punished.

If low-carb fits that description for you, use it. If it doesn't, stop letting the internet convince you that you're doing something wrong by eating potatoes.
